Care Navigator / Receptionist

An exciting opportunity has arisen for a Care Navigator / Receptionist to join our busy team, working at both our Lofthouse Site and The Manse, our Rothwell Site. The current vacancy we have available is for 30 hours a week over a 5 day period (Monday to Friday) - 6 hours a day from 12:30 - 18:30.

The practice working day is between 07:45 and 18:30, the successful applicant will be willing to work flexibly when needed. Interviews to be held week commencing 2nd November.

Care Navigators are the first line of contact with our patients and visitors. This is a varied role that includes face to face and telephone contact with patients as well as administrative duties.

You will work as part of a team of Care Navigators, maintaining a high quality and efficient service for the patients, Doctors and Nurses. Reception experience/customer service is desirable, but training will be given to the successful candidate.

To receive, assist and direct patients in accessing the appropriate service or healthcare professional, in a courteous, efficient, and effective way. Provide general assistance to the Practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone.

Act as a Care Navigator, signposting patients to appropriate NHS services or appropriate Health Professional to meet their needs. In addition, the post holder will also support the multi-disciplinary team, providing administrative support as required, in line with the needs of the organisation.
